    Bank Statement last 6 month
    Insurance
    Ticket.
·         If you have been refused a visa by an Embassy or High Commission in the last 5 years: Written explanation about reason for trip and refusal

    2 Photo (35 x 45 mm white background with dark shirt face size 25 x35 mm glossy finish),
    Covering letter on company letterhead address to Czech Embassy-New Delhi.( mention with passport no.travel dates.)
    Return Ticket,
    Exchange/ credit card copy,
    Last 6 month bank statement,
    Last 3 months salary slips ( Not mandatory)
    Hotel Confirmation with day to day itinerary.
    Leave letter from company
    Insurance
    For student school/ college Id card copy or Bonafide cert

    If minor traveling with single parent or alone then Notarized NOC letter on Rs. 100 stamp paper signed by both parents is required.
